Treatment. I consent to assessment and, where clinically appropriate and agreed at the visit, to treatment by Avara Medical. I understand a consultation does not guarantee that a procedure will be carried out on the same day.

Risks. I understand that all medical and aesthetic procedures carry risks, including but not limited to bruising, swelling, asymmetry, infection and an outcome that differs from what was hoped for. These will be discussed with me before any treatment.

Photography. I consent to clinical photographs being taken for my medical record. Any use beyond my record — including marketing — will only occur with my separate, explicit permission.

Confidentiality. My information is held in confidence and used to provide and improve my care, in line with the practice's obligations.

Liability. I confirm the medical information I have given is accurate and complete to the best of my knowledge, and I will inform the practice of any change.

Payment. Fees are quoted in US dollars and converted at a fixed rate of US$1 = J$160. Cash, credit and debit cards, and bank transfer are accepted.

Deposits. A 50% deposit confirms the appointment. The deposit is non-refundable and is applied to the visit.

Cancellations. Cancellations require eight hours' notice. Cancelling inside eight hours incurs 50% of the consultation fee, and a missed appointment without notice is charged in full.
